@EastSgDad and @QinWei: I take it out from the top of my shorts most of the time. I find it easier that way. 

You do not need to pull down very low. Just enough to take your penis out. However, I have seen guys who also take out his balls and penis. And as QinWei mentioned, some even pull it down exposing his full butt cheek. The most extreme one will pull his shorts/pants down all the way until it drops below his thigh. 

All ways is ok as long as you are comfortable with it and you could pee without wetting your shorts/pants or the floor.
